Insurance coverage
We currently accept the following insurance plans:

We bill your insurance directly. You'll only be responsible for your copay or any applicable deductible.
Verify your coverage
Insurance coverage can vary by plan. We recommend contacting your insurance provider before your first appointment to verify:
Mental health and primary care coverage
Copay or coinsurance amounts
Whether a referral is required (typically not needed for our services)
Deductible status
If you have questions about your specific coverage, we're happy to help verify benefits before your appointment.
Self-pay options
If you don't have insurance or prefer to pay out-of-pocket, we offer straightforward self-pay rates:
Initial Appointment: $120
Follow-Up Appointments: $60
Payment: We accept all major credit cards, Health Savings Accounts (HSA), and Flexible Spending Accounts (FSA).
Why self-pay? Some patients choose self-pay for privacy, flexibility, or because their insurance doesn't cover telehealth services.
Insurance verification & patient responsibility
It is your responsibility to verify your insurance coverage before your appointment. While we make every effort to verify benefits, insurance coverage can be complex and subject to change.
You are responsible for:
Confirming that MLR Holistic Health is in-network with your specific plan
Understanding your deductible, copay, and coinsurance amounts
Knowing which services are covered under your plan
Verifying that psychiatric and mental health services are included in your benefits
Understanding any pre-authorization or referral requirements
Providing accurate and current insurance information
Please note: Insurance benefits quoted by your insurance company or our office are estimates only and not a guarantee of payment. Your insurance company makes the final determination of coverage and payment.
What happens if insurance denies a claim
If your insurance company denies a claim or pays less than expected, you are responsible for the full balance.Common reasons for claim denials include:
Services not covered under your specific plan
Deductible not yet met
Out-of-network benefits lower than expected
Pre-authorization not obtained
Plan limitations on frequency of visits
We will work with you to appeal denied claims when appropriate, but you remain financially responsible for all services provided.
Payment & billing policies
Payment is due at the time of service unless other arrangements have been made in advance.

Late payment: Accounts more than 30 days past due may be subject to a late fee and/or collection efforts.
Refund Policy
Prepaid appointments: If you prepay for an appointment and provide at least 24 hours notice of cancellation, you will receive a full refund. Cancellations with less than 24 hours notice are subject to our cancellation policy and may not be refunded.
Insurance overpayments: If you overpay due to insurance processing, we will issue a refund within 30 days of receiving payment from your insurance company.
Services rendered: We do not provide refunds for services that have been rendered. If you are dissatisfied with care, please contact us to discuss your concerns.
Missed appointments & late cancellations
Please see our Cancellation Policy for detailed information about our 24-hour cancellation notice requirement.
Missed appointments and late cancellations (less than 24 hours notice):
Are not covered by insurance
Will be charged at the full self-pay rate
Payment is due immediately
May result in discharge from the practice if it becomes a pattern
